Dear Information Coordinator,
 
Under the Freedom of Information and Protection of Privacy Act, please provide me with
·         Two of the most recently completed inspection reports for the Sunrise of Vancouver care facility, within the time frame of 2013-2016.  
 
·         Information regarding the specific number of complaints that have led to follow ups within that same time frame, as well as all documentation regarding the most recent complaint and follow up.

If possible, please send the documents in electronic format to my email address.
If you have any concerns, please phone me at (604) 787 7253.
Please contact me when the information requested is able to be released.

Explanation:

I wrote my request to this particular nursing home in order to satisfy personal curiosity. This was the nursing home that my elderly aunt stayed at for the last few years of her life, and I remember hearing a lot of bad stories from her about it (typically regarding the staff). I figured that filing an FOI request would help me to determine whether it was actually as bad as she claimed. I was unable to find much about the facility via research, but did find an article in the Sun describing staff who mistreated clients at another location. There is no name given for the coordinator in this request because it is addressed to Vancouver Coastal Health, which lists an FOI mailing address on their website instead.

  2. I really like how this FOI request has a personal reasoning behind it, and I believe that it's very important to find out any history of abuse there could be when trying to find a reputable and safe home for our loved ones.
